Claiming From CBHS

Claiming from CBHS is simple, fast and convenient.

There are many ways to claim with CBHS.

For Extras services, some service providers may have HICAPS or IBA facilities, which means you can swipe your CBHS membership card at the time of service, and the claim is automatically processed; you just pay the 'gap'.

If your Extras provider does not have HICAPS or IBA facilities, you may be able to claim online, depending on the service and the fee charged. Register for CBHS Online Member Service Centre, and follow the links to Online Claiming- eClaim.

You can also fill out a claim form and send it to us for fast processing.

Claim forms can be downloaded by clicking here or can be ordered from the Member Care Centre by calling 1300 654 123.
 
Complete your claim form, attach the receipt(s)/account(s), and send it to:

  • CBA Internal Mail - CBA 2395 001, CBHS Health Fund Limited, 10 Pitt Street Parramatta
  • Post - Locked Bag 5014, Parramatta NSW 2124
  • Fax - 02 9843 7676
  • Email - help@cbhs.com.au

Once received by CBHS, your claim will be processed and payment of the benefit made either as a deposit to your nominated bank account (if you have already paid for service), or as a cheque made out to your health service provider. Full details of the payment will also be emailed or mailed to you.
