No, because Fitness Passport is a CBA employee benefit. If you’re not a current CBA employee, but you hold CBHS Extras cover, you may be able to claim a benefit on your gym membership or personal training. Complete the CBHS Health Management Program Authorisation (HMPA) Form together with your healthcare professional and return to us.
No, you don’t need to be the main policy holder. You’ll just need to ensure you maintain an active CBHS membership on eligible cover with premiums paid up to date.
Yes, CBHS members with Extras cover can claim benefits on the cost of Fitness Passport, just like you would for a gym membership. You’ll need to submit a Health Management Program Authorisation (HMPA) Form completed by your GP, specialist, or allied health service provider, along with your claim.
You can easily change your employee pricing within the Fitness Passport portal. Simply update your membership by selecting the relevant ‘Commonwealth Bank CBHS member’ option.
Absolutely! When you join CBHS on an eligible cover, you can easily change to the ‘Commonwealth Bank CBHS member’ pricing on the Fitness Passport sign up portal.

If you’re both current CBA employees and CBHS members, you’re both eligible for the CBHS exclusive membership pricing. It can only be applied once to each Fitness Passport membership, however. You could take out either a single or family Fitness Passport membership, depending on your needs.
