Submit your claims to Medicare by using this form.
Use this form for service providers who do not participate in Access Gap and bill you directly. This form allows Medicare
to forward your claim to us for payment of remaining benefits after they have processed your claim.

FlexiSaver (Basic Plus)
FlexiSaver (Basic Plus) is an entry level package for the young healthy singles and couples, designed to give the flexibility to use Extras overall limit on service needed the most while saving money by accepting to have exclusions on hospital treatments not needed at this stage of life.
KickStart (Basic Plus)
CBHS’ KickStart (Basic Plus) package is an affordable level of cover designed for the fit and healthy, because accidents do happen. It includes general benefits for Extras services, wellness benefits and more.
StepUp (Bronze Plus)
StepUp (Bronze Plus) is a mid-level package cover option that incorporates Intermediate Extras and Limited Hospital (Bronze Plus). It is suitable for young families and those who want to access benefits for popular services.
Prestige (Gold) is the highest level of packaged cover, incorporating generous benefits for a broad range of services.
Basic Plus Hospital
Basic Plus Hospital cover is suitable for those who want to be covered for the basics and to be able to choose their own doctor or specialist when receiving treatment in a public hospital.
Limited Hospital (Bronze Plus)
Limited Hospital (Bronze Plus) cover is CBHS’ intermediate-level hospital cover. This product is suitable for those who want to be covered for the essential services and items and not services that they are less likely to need.
Active Hospital (Silver Plus)
Designed for young professionals and empty nesters, Active Hospital (Silver Plus) is a high level product that covers more than the basics while excluding some services you are less likely to need - like pregnancy.
Comprehensive Hospital (Gold)
CBHS’ Comprehensive Hospital (Gold) cover offers the highest level of hospital cover available, and is designed for those who want cover for unexpected situations.
Essential Extras offers Extras cover at an affordable price. Included are benefits for preventative dental, wellness benefits and basic cover for dental, optical, chiropractic and physiotherapy services.
Intermediate Extras is suitable for those who want benefits for common services and treatment. It includes generous service limits, cover for preventative dental services and wellness benefits.
Top Extras is CBHS’ premium Extras product, providing members with generous coverage for a comprehensive range of services and treatments.

CBHS CLOSED PRODUCTS
Product sheets for CBHS closed products
The following products are closed to new members and transferring members. They remain available to members who were covered at the time of product closure, and have not since upgraded or transferred to a new policy.
HELPER Registration Form
CBHS HELPER Registration Form is used to authorise hospitals and people to access CBHS’ Hospital Extranet Link for Patient Eligibility Records. The HELPER system gives named individuals access to private patient eligibility individual over the internet.
Authorisation to Release Information Form
Use this form to authorise the release of the patient and member’s details by the patient’s doctors, hospitals and any other authorities to CBHS and CBHS’ medical consultants. The patient (or member if the patient is a dependent on their parent’s membership) must sign the form for the authorisation to take effect.
Certificate for Medical Practitioner
Members may need to submit this form if they have joined or upgraded their level of hospital cover in the past 12 months. This medical report allows CBHS’ medical advisors to check whether the member’s condition is a pre-existing condition and determine whether it is covered by the member’s cover.
The Accident/Injury/Condition Form allows CBHS to access the member’s claim in relation to an accident, injury or condition. You’ll need to outline the nature of the injury or condition and provide other relevant details such as date of occurrence and whether the injury occurred at work.
Hospital Special Consideration Forms
On some occasions, CBHS may need to obtain information directly from providers such as hospitals, doctors and other health professionals. These are the forms used for HELPER registration, hospital payments, hospital special consideration, information release authorisation, medical practitioner certificates and accident/injury/condition forms.
